Quick Questions to Bring to Your Doctor's Office

Oftentimes, a shift in your loved one's typical behavior or habits can indicate an issue with depression. If you notice changes happening, gently and supportively help identify the issues with them, then print out and use the following list as a starting point to talk with their care provider.

With help from your loved one, circle the symptoms below that best describe the changes in their lives.

1. I am not sleeping well.

  • I have trouble falling asleep.
  • I have trouble staying asleep.
  • I have been sleeping more than usual.
  • This issue has occurred for more than two weeks.
2. I have been excessively tired (even after a night's sleep).

3. I have less desire or need to eat.

4. I am eating more than usual.

5. My weight has changed (either decreased or increased).

6. I am not having any fun.

7. I am having difficulty concentrating, remembering or making decisions.

8. I have had thoughts of death or suicide, or I have attempted suicide.

9. I have felt socially withdrawn.

  • This issue has occurred for more than two weeks.
10. I am experiencing unusual guilt and/or excessive feelings of worthlessness, hopelessness or helplessness.

11. I feel life is not worth living or that my family would be better off if I were no longer alive.
